Finally got some time to post some comments & fixes and rather than ramble on as I tend to do i'll try & make a bullet list to get right to the point

What to fix:

GEAR:

-Off hand - TERRIBLE, a fast offhand is bad, and has been since the implementation of Threat of Thassarian

-WAY over expertise cap (by about 7 expertise on your mainhand)

-You could probably shed a little hit if you have some gear to swap around

-Cloak enchant should be +23 Haste

-You placed a nightmare tear in your shoulders for a +4 str bonus when there are other places you could have put it for a better bonus(chest would probably be the best bet)

-generally raiders prefer Tuskarr's Vitality over Icewalker for better movement in most boss fights(especially for things like cutter beam)

- I see you're in the "Gem All Strength" camp and while it's the easy way to go you can(depending on the appeal of socket bonuses) gem haste/str in yellow sockets(since you don't need hit)

SPEC:

Overall pretty good with only 1 flaw

why do you have a point in Night of the dead? it would likely be better spent in chill of the grave(to accelerate RP gain at the start of fights & fuel frost strikes for when KM is up

the second point is minor & more of a future plan, as you gain ground on the tanks gear wise(i assume if you raid in either KB or salmon you're in mostly 251-264 gear & the tanks are in 277) you may need to think about putting a point in Subversion for the threat reduction.  I had to do it for 10man raiding & ocasionally for 25.

UI & LAYOUT:

Looking at your screenshot doesn't tell much about a raid setting, do you use any mods to watch procs(Rime & Killing Machine), do you use a raidframe mod? what do you use to track diseases on your target?

That's about all i cvan think of at the moment, i'll post more if i think of anything else.
